Scorecards track data to measure progress toward our community outcomes. Each scorecard consists of objectives and measures. Each measure has a trend, duration, target value and current value. Below you see Monterey County's Community Health Assessment (CHA) Scorecard.
A Community Health Assessment (CHA) provides comprehensive information about the community’s current health status, needs, and issues. In turn, this information can help with developing a community health improvement plan by justifying how and where resources should be allocated to best meet community needs (CDC, 2018).
